Why This Role Matters – The Impact of a Skilled LVN in Tele‑Rehabilitation As a Licensed Vocational Nurse (LVN) on our remote, day‑shift team, you will be a pivotal link between patients, physicians, and the multidisciplinary care team. Key Responsibilities – What You’ll Do Each Day Under the direct supervision of a Registered Nurse (RN), you will:

  • Provide patient‑centered nursing care across the full spectrum of adult, geriatric, and, when needed, pediatric populations.
  • Administer oral and intravenous medications accurately, adhering to physician orders and institutional protocols.
  • Document all assessments, interventions, and outcomes promptly in the electronic medical record (EMR) using clear, concise language.
  • Measure and record vital signs, intake and output, and other critical physiological data.
  • Perform wound care, including dressing changes, and assess healing progress.
  • Manage ostomy sites, ensuring proper care and patient education.
  • Insert, maintain, and monitor nasogastric (NG) and gastrostomy tubes, delivering nutrition and medications as ordered.
  • Insert and care for urinary catheters, monitor for signs of infection, and maintain appropriate documentation.
  • Provide tracheostomy and ventilator care, including suctioning, circuit checks, and emergency response.
  • Collect and label specimens (blood, urine, sputum) for laboratory analysis, following infection‑control standards.
  • Collaborate with the RN and interdisciplinary team to develop and update individualized care plans, discharge plans, and patient education materials.
  • Identify changes in patient condition, promptly notify the RN and physician, and document all communications.
  • Participate in multidisciplinary rounds, contributing observations and insights that influence treatment pathways.
  • Support non‑licensed staff by providing guidance, training, and motivation to ensure cohesive, high‑quality patient care.
  • Engage in continuous quality improvement initiatives, reporting any adverse events or near‑misses in accordance with hospital policy.

Essential Qualifications – What You Must Bring To be successful in this role, you must meet the following minimum criteria:

  • Current LVN/LPN licensure in the state of Texas (or ability to obtain licensure within 30 days of hire).
  • Valid Basic Life Support (BLS) certification at the time of employment.
  • High school diploma or GED; completion of an accredited nursing program (LPN/LVN) is required.
  • Excellent written and verbal communication skills in English, including proficiency with medical terminology and abbreviations.
  • Demonstrated ability to accurately chart in an EMR system, ensuring compliance with regulatory standards.
  • Strong organizational and time‑management skills to prioritize tasks in a fast‑paced, remote environment.
  • Ability to work collaboratively with RNs, physicians, therapists, social workers, and other members of the care team.

  • Proficiency with advanced devices such as ventilators, tracheostomy kits, PICC lines, and feeding tubes.
  • Additional certifications (e.g., Advanced Cardiovascular Life Support (ACLS), Wound Care Certification) that enhance clinical expertise.
  • Two or more years of direct patient‑care experience, reflecting a deeper understanding of patient flow and interdisciplinary collaboration.
