Part Time/On Call Chronic Care Manager - LPN/CMA - Remote

Bluefish Medical is looking for LPN’s and CMAs in Alabama, Arkansas, Florida, Georgia, Mississippi, Tennessee, Texas, Kentucky, Oklahoma, or Virginia. Chronic Care Management (CCM) provides one on one attention via telephone calls to patients with two or more chronic diagnoses. The Chronic Care Manager collects patient health information to identify risks, provide education and encourage compliance of treatment. The Care Manager will be responsible for creating a patient specific Care Plan that provides goals and interventions to implement, monitor and support the needs of the patient as defined by the PCP. The CCM program supports the patient to promote the highest level of quality care. LPN’s can earn up to $24 per hour and CMA’s can earn up to $17 per hour. Requirements include:

• Ability to work 20 hours minimum during weekdays between 8:00 AM and 5:00 PM CST and take the after hours on-call responsibilities 2-3 weeks per month. (On Call weeks worked earn $150 per week bonus in addition to time worked)
• LPN or CMA required.
• Must have high speed internet, not satellite internet or hotspot.
• Must be proficient in Microsoft Outlook and Excel
• Familiar with documentation in EMR's
• Must be self-motivated and able to work in a remote environment and love helping others.

Responsibilities include:

• Provide initial risk assessment gathering the clinical elements necessary to determine a patient specific Care Plan.
• Work with patients to plan and monitor their care as determined by the PCP.
• Promote compliance with PCP/Specialist office visits.
• Promote compliance with medication.
• Act as a patient advocate, liaison, and information resource.
• Provide chronic care education for chronic/complex conditions as determined.
