Key Responsibilities:

  • Deliver professional nursing care to patients in accordance with established plans of care
  • Conduct comprehensive assessments, develop individualized plans of care, and provide skilled interventions to achieve realistic outcomes
  • Ensure compliance with federal and state regulations governing hospice care services
  • Provide education and support to patients, families, and caregivers
  • Collaborate with our clinical team to achieve exceptional patient outcomes and experiences
  • Maintain accurate and thorough documentation of patient care and progress

Requirements:

  • Graduate of an accredited school of nursing, Bachelor's degree preferred
  • Current Registered Nurse licensure (in good standing) in the state(s) of practice
  • One (1) year of experience as a nurse in hospice care or post-acute setting
  • Current CPR certification and valid driver's license
  • Strong clinical skills, excellent communication and interpersonal skills, and ability to work in a fast-paced environment
