As a key member of our Landmark product team, you'll play a vital role in delivering home-based medical care to complex, chronic patients, empowering older adults to spend more days at home. Our integrated care model addresses the physical, mental, and social needs of individuals, ensuring seamless health journeys across care settings. In this role, you'll work alongside our call center clinical team, supporting patients with acute changes in condition and assisting with administrative requests as needed. As a Triage Nurse, you'll assess patient needs, determine next steps in care, and provide support for patient outreach and engagement, scheduling, and other administrative functions. Your expertise will be essential in driving patient care, and you'll be available to address patient concerns during daytime, overnight, weekend, and holiday shifts.

We're looking for a highly skilled and adaptable nurse who can learn new software systems, efficiently collect information from callers, and follow guided protocols. You'll be an integral part of our team, promoting a customer service-oriented approach to care delivery, supporting our cultural platforms, and ensuring compliance with departmental standards and policies.

Primary Responsibilities:

  • Provides clinical assessment and intervention utilizing the nursing process
  • Collaborates with Landmark staff to ensure favorable patient outcomes
  • Documents patient interactions, assessments, plans of care, and interventions in the patient record
  • Participates in team member orientation, in-services, staff meetings, and educational conferences
  • Develops and achieves professional growth goals and objectives
  • Promotes a customer service-oriented approach to care delivery and supports Landmark cultural platforms

Secondary Responsibilities:

  • Triage clinical calls and email requests
  • Provide warm handoffs to advanced clinicians or physicians as needed
  • Schedule urgent visits and request urgent visits as necessary
  • Manage patient care, including medication reviews, plan of care reviews, and immunization questions
  • Collaborate with interdisciplinary teams, including LM1 APCs, on-call providers, and community providers
  • Document and communicate in real-time, reviewing charting software dashboards daily

Required Qualifications:

  • Bachelor's degree in Nursing
  • Preferred residence in a compact state, with licensure in compact states within 6 months
  • Ability to obtain licensure for non-compact states within 6 months
  • Pre-existing licensure with 2+ states (with no current/pending restrictions)
  • ER, urgent care, or ICU experience required; preferred within the past 1-2 years
  • Thorough knowledge of professional nursing principles, nursing diagnosis, and medical supplies
  • Proficient computer skills, including troubleshooting and adapting to multiple applications
  • Excellent verbal, written, and interpersonal communication skills
  • Proven ability to work as a team player, remain calm in high-stress situations, and demonstrate critical thinking and crisis intervention skills

We offer a competitive salary range of $27.07 to $53.08 per hour, depending on location, experience, and qualifications.
